The Halo: Campaign Evolved Forgotten Terminal is found in the Halo mission.

How to find this terminal

Forgotten is fairly early in the mission. While driving the Warthog through the underground tunnel, stop after you round this corner. Climb over the slanted metal half-wall and drop onto the suspended platform. Do not jump all the way down to the open floor. Once you land, turn the camera around 180 degrees and you will see the terminal hiding in the shadows against the wall.
